Current git produces the same result, except the crash now also gives a dsound
fatal error:
err:dsound:DSOUND_MixOne Fatal error. Under/Overflow? primary_done=3076,
mixpos=262792/576784 (60360/132480), primary_mixpos=20376, writepos=17300,
mixlen=0
As such, I'm attaching a full +dsound log.

I'n not sure if he meant only mac, or both mac/linux (cider/cedega) clients
though.
Michael Jenkins reported above that issue isn't applicable to cedega-wrapped
client, could anybody confirm that?

MDKDEM3D.EXE the Direct3D renderer nolonger crashes with
wine-1.1.2-551-g34aa6b7, now the first level loads, the sky/background is drawn
and the playermodel, but not the ground or walls.
MDKDEM95.EXE the ddraw renderer is the same, game works but screen only shows
black.

I had the problem of the invisible window and a high CCU load when starting
dreamweaver. By this text passage (in the description) I found out that I had
to delete the drive assembly allocation for drive z.
Then it works fine.
"(though you have to hit Enter to get an invisible
dialog box to go away; that's the infamous z-order problem)"
I hope that it is possible to help others with the same problem.
For me it was hard to find the solution.
